Since its inception in October 1983, the Shanghai Changning Notary Public Office has developed over 40 years to become a significant legal service institution in the city. Based in this district and covering its service across the city, it provides quality notary legal services to citizens, enterprises, and institutions. The Office constantly innovates its service models in the fields of judicial assistance, notarization digitalization, and public legal services, serving the people's livelihood and businesses.

Since 2018, it has been extensively assisting the district courts in judicial assistance matters. When it launched the Remote Notarization Video Consultation Service in 2015, it tapped the Internet-based video communication, making it available to daily notary legal consultation services for the community. In 2019, with its online application system, it unveiled the “Notarization + Internet” service to accept notarization applications 24/7. In 2022, the “Changning Notary Preservation” electronic preservation platform was introduced. In 2023, the “Notary Service at Your Doorstep” project was selected as an effort in the “Rule of Law for the People” project in Shanghai, becoming the first in the city to visit neighborhood committees in a comprehensive manner. In the same year, it pioneered the “Court + Notary + Civil Affairs” tripartite construction model in Changning, jointly building a complete “service chain” for family legal matters.

Since 1999, it has been awarded the title, “Shanghai Model Institution” many times. I. Notarization for Birth

※Notarization for birth refers to the activity that the Office certifies the legal fact of the client's birth upon the client's application.

The materials that the applicant shall submit include:

(I) The applicant's identification certificate

1. In case of a domestic natural person:

(1) Identity card

(2) Household register

2. In case of an overseas natural person:

(1) Original household register or domestic household register cancellation certificate

(2) Identity card (for the person whose household register has not been canceled)

(II) Identification certificates of the applicant's parents

1. If both parents are alive:

(1) Identity cards of the parents

(2) Householder registers of the parents

2. If one of the parents or both parents are dead:

(1) Death certificate issued by hospital

(2) Household register cancellation certificate issued by the police station where the household register is maintained

(III) Marriage certificate of the parents

If the parents have divorced, the divorce certificate issued by the bureau of civil affairs and the divorce agreement

or the paper of civil mediation issued by people's court

or the paper of civil judgment and the certificate of validity of judgment issued by people's court

(IV) The applicant's birth certificate and one-child certificate

(V) The applicant's reported birth registration certificate

(VI) The proof of kinship (to be completed by the applicant's or the parents' personnel dossier custody department)

(VII) Photo: Recent two-inch bareheaded photo; the number of the photos = the number of the notarial certificates + 1

(VIII) If a proxy acts for the applicant, a power of attorney and the proxy's identification certificate are required additionally

Important Notes:

1. The applicant needs to meet any of the following conditions:

(1) The applicant was born in the city (Shanghai)

(2) The applicant has his/her domicile or habitual residence in the city (Shanghai)

2. For the people from other countries, a notarization to prove that "the copy of the medical certificate of birth is consistent with the original" is applicable.

II. Notarization for Marriage Status

※Notarization for Marriage Status refers to the activity that the Office certifies the legal fact of the client's marriage status upon the client's application.

The materials that the applicant shall submit include:

(I) The applicant's identification certificate

1. In case of a domestic natural person:

(1) Identity card

(2) Household register

2. In case of an overseas natural person:

(1) Original household register or domestic household register cancellation certificate

(2) Identity card (for the person whose household register has not been canceled)

(II) Marriage status

1. Married:

(1) Marriage certificate

(2) Photo: Recent two-inch bareheaded photo of both male and female; the number of the photos = the number of the notarial certificates + each for 1

2. Divorced

(1) Divorced certificate

(2) Divorce agreement

or the paper of civil mediation issued by people's court

or the paper of civil judgment and the certificate of validity of judgment issued by people's court

(III) If a proxy acts for the applicant, a power of attorney and the proxy's identification certificate are required additionally

Important Notes:

1. The applicant needs to meet any of the following conditions:

(1) The applicant got married or divorced in the city (Shanghai)

(2) The applicant has his/her domicile or habitual residence in the city (Shanghai)

(3) Non-married Status (Divorced Status, Widowed Status) shall be declared by the applicant in person.

2. For the people from other countries, a notarization to prove that "the copy of the Marriage certificate, Divorced certificate, paper of civil judgment or paper of civil mediation is consistent with the original" is applicable.

III. Notarization for Kinship

※Notarization for Kinship refers to the activity that the Office certifies the legal fact of the kinship between the applicant and the relative upon the client's application.

The materials that the applicant shall submit include:

(I) The applicant's identification certificate

1. In case of a domestic natural person:

(1) Identity card

(2) Household register

2. In case of an overseas natural person:

(1) Original household register or domestic household register cancellation certificate

(2) Identity card (for the person whose household register has not been canceled)

(II) Identification certificates of the relatives (same as the applicant)

(III) Kinship supporting materials (such as marriage certificate, birth certificate, one-child certificate, household registration certificate, archives)

(IV) If a proxy acts for the applicant, a power of attorney and the proxy's identification certificate are required additionally

Important Notes:

1. The applicant needs to meet any of the condition:

The applicant has his/her domicile or habitual residence in the city (Shanghai)

2. For proving the kinship fact the applicant shall provide 2-3 supporting materials issued by different department.

IV. Notarization for Existence of Criminal Record or No Criminal Record

※Notarization for Existence of Criminal Record or No Criminal Record refers to the activity that the Office certifies the legal fact of the client's existence of criminal record or no criminal record during the residence in the P.R.C. upon the client's application.

The materials that the applicant shall submit include:

(I) The applicant's identification certificate

1. In case of a domestic natural person:

(1) Identity card

(2) Household register

2. In case of an overseas natural person:

(1) Original household register or domestic household register cancellation certificate

(2) Identity card (for the person whose household register has not been canceled)

(II) The certificate of “no criminal record” issued by the residential police station

(III) A paper of criminal judgment issued by people's court

(IV) If a proxy acts for the applicant, a power of attorney and the proxy's identification certificate are required additionally

Important Notes:

1. The applicant needs to meet any of the following conditions:

(1) Domestic natural person has his/her domicile or habitual residence in the city (Shanghai)

(2) Overseas natural person has his/her domicile or habitual residence in the city (Shanghai) and applied the certificate of “no criminal record” issued by the residential police station.

2. This notarization is valid for 6 months, please be aware of the time efficient.

V. Notarization for Driving License

※Notarization for Driving License refers to the activity that the Office certifies the legal fact of the client's driving license of P.R.C. within the valid period and vehicle type permitted to drive upon the client's application.

The materials that the applicant shall submit include:

(I) The applicant's identification certificate

1. In case of a domestic natural person:

(1) Identity card

(2) Household register

2. In case of an overseas natural person:

(1) Original household register or domestic household register cancellation certificate

(2) Identity card (for the person whose household register has not been canceled)

(II) Driving License within the valid period

(III) If a proxy acts for the applicant, a power of attorney and the proxy's identification certificate are required additionally

Important Notes:

1. The applicant needs to meet any of the following conditions:

(1) The driving license was issued in the city (Shanghai)

(2) The applicant has his/her domicile or habitual residence in the city (Shanghai)

VI. Notarization for Academic Credential or Degree

※Notarization for Academic Credential or Degree refers to the activity that the Office certifies the legal fact of the client's graduation (completing study) and getting relevant certificate upon the client's application.

The materials that the applicant shall submit include:

(I) The applicant's identification certificate

1. In case of a domestic natural person:

(1) Identity card

(2) Household register

2. In case of an overseas natural person:

(1) Original household register or domestic household register cancellation certificate

(2) Identity card (for the person whose household register has not been canceled)

(II) Certification materials

1. Diploma, Degree Certificate, Certificate of Studying

2. experience certificate (filling by personnel dossier custody department)

(III) Photo: Recent two-inch bareheaded photo; the number of the photos = the number of the notarial certificates + 1

(IV) If a proxy acts for the applicant, a power of attorney and the proxy's identification certificate are required additionally

Important Notes:

1. The applicant needs to meet any of the following conditions:

(1) The applicant got the Diploma, Degree Certificate or Certificate of Studying issued in the city (Shanghai)

(2) The applicant has his/her domicile or habitual residence in the city (Shanghai)

2. For the people from other countries, a notarization to prove that "the copy of Diploma, Degree Certificate, Certificate of Studying or transcript is consistent with the original" is applicable.

VII. Notarization for Copy of Other Document

※Notarization for Copy of Other Document refers to the activity that the Office certifies the legal fact of the client's document copy is consistent with the original upon the client's application.

The materials that the applicant shall submit include:

(I) The applicant's identification certificate

1. In case of a domestic natural person:

(1) Identity card

(2) Household register

2. In case of an overseas natural person:

(1) Original household register or domestic household register cancellation certificate

(3) Identity card (for the person whose household register has not been canceled)

(II) Document need certified

(III) If a proxy acts for the applicant, a power of attorney and the proxy's identification certificate are required additionally

(1) The applicant got the document or certificate in the city (Shanghai)
